Trademark FormsThe two types of trademark forms that are most commonly requested are trademark licenses and trademark assignments. Here's a standard trademark assignment. Unfortunately, trademark licenses are somewhat complex and, if not written correctly, can invalidate rights in the mark, so we have no standard "form" for a licensing transaction.
