An Invention Disclosure Form is used to record inventions and ask questions necessary for firstly, deciding whether and how to file for patent protection, and secondly, preparing the patent application.  Many different formats of invention disclosure forms exist for different situations.  We like this one as being generally applicable to many different situations.
